Job summary

Qualifications

• Two years experience in a skilled nursing facility rehabilitation environment preferred.

Responsibilities

• Contributes to a positive work team by sharing information and using problem-solving methods.
• Provides treatments to meet patient needs following Rehabilitation Services guidelines.
• Maintains accurate documentation for regulatory, licensing, and payor requirements.
• Collaborates with Rehabilitation Director, therapists and other disciplines to manage clinical cases effectively.
• Demonstrates safe working practices and professional conduct.
• Supports committees and project work within the system as needed.
