Job summary
Woodbridge Care Center in Tampa, FL is seeking a compassionate Occupational Therapy Assistant (OTA) to join our rehabilitation team. The OTA will collaborate with an interdisciplinary team to deliver evidence-based OT services, focusing on ADL improvement and functional outcomes for long-term care residents.
Ideal candidates have an OTA license, graduation from an accredited program, and familiarity with PDPM and Medicare documentation.
Qualifications
- Current Florida OTA license in good standing.
- Graduated from an accredited OTA program.
- Experience in skilled nursing or long-term care preferred.
- Knowledge of PDPM and Medicare documentation requirements.
Responsibilities
- Develop OT treatment plans with the OT, deliver evidence-based interventions.
- Support ADL improvement, fall prevention, and functional maintenance.
- Coordinate care with OT, PT, SLP, nursing, and physicians.
- Maintain timely, accurate documentation for medical necessity and progress.
- Participate in care conferences and quality improvement initiatives.
- Monitor progress and adjust plans to improve outcomes.
