Job summary

Responsibilities include patient assessments, creating individualized treatment plans, guiding therapeutic exercises, patient/family education, and collaboration with healthcare professionals to ensure comprehensive care.

Qualifications

• Outpatient/home health experience required.
• Strong knowledge of anatomy and physiology, plus medical terminology.
• Proficiency in Activities of Daily Living (ADLs) and patient care.

Responsibilities

• Conduct patient assessments to determine individual needs and goals.
• Develop personalized treatment plans to improve patients' abilities to perform daily activities.
• Provide therapeutic exercises and activities to enhance motor skills and coordination.
• Educate patients and their families on adaptive techniques and equipment.
• Collaborate with healthcare professionals to ensure comprehensive patient care.
