Sigma Health Rehab is seeking a weekend Occupational Therapy Assistant (COTA) to work in a Skilled Nursing Facility in Canandaigua, NY. The OTA/COTA will support therapy services under the supervision of an OT and participate in a collaborative care team.

Qualifications

• Graduate of an accredited OTA program is required.
• Licensed to practice OTA in NY is required.

Responsibilities

• Select therapy activities per supervising OT plan of care.
• Assist residents with daily activities.
• Monitor performance and document progress.
• Teach residents to use assistive equipment.
• Collaborate with OT to set rehabilitation plan.
• Maintain cooperation within the healthcare team.

Education

Duties of an exceptional therapist include:

• Select therapy activities that follow the plan of care set out by the supervising occupational therapist


• Assist residents in carrying out daily activities.


• Monitor residents' performance in therapy activities, provide encouragement and document on residents as needed.


• Teach residents how to use special equipment to assist with daily function.


• collaborate with the OT to establish a rehabilitation plan.


• Maintains a cooperative relationship among healthcare team members & contributes to team effort

COTA Requirements Include:
