Responsibilities

• Follow and modify the therapist’s plan of care based on patient response and document communication with the evaluating therapist.
• Implement occupation-based, evidence-informed interventions tailored to patients’ needs.
• Adapt treatment activities considering environmental factors and patient limitations.
• Collaborate with patients, families, caregivers, and healthcare team members throughout the therapy process.
• Educate patients, families, and colleagues using effective teaching methods.
• Complete accurate documentation supporting clinical decisions and treatment progress, including daily notes and EMR entries.
• Participate in quality improvement activities and communicate progress with team and external providers.
• Perform treatment procedures such as range of motion assessments, ADL training, ultrasound, and fluidotherapy.
• Maintain compliance with program requirements and communicate treatment recommendations to referring providers.

Key Responsibilities

• Follow and modify the therapist’s plan of care based on patient response, ensuring documentation of communication with the evaluating therapist.
• Implement occupation-based, evidence-informed interventions tailored to patients' physical, cognitive, and emotional needs.
• Adapt treatment activities considering environmental factors and patient limitations, and adjust approaches to achieve desired outcomes.
• Collaborate with patients, families, caregivers, and healthcare team members throughout the therapy process.
• Educate patients, families, and colleagues using effective teaching methods.
• Complete accurate, professional documentation supporting clinical decisions and treatment progress, including daily notes and EMR entries.
• Participate in quality improvement activities and communicate effectively with team members and external providers regarding patient progress.
• Perform specific treatment procedures such as range of motion assessments, ADL training, ultrasound, and fluidotherapy.
• Maintain compliance with relevant program requirements and communicate treatment recommendations to referring providers.
