Corewell Health in Grand Rapids, MI is seeking an Occupational Therapy Assistant to support therapists in delivering comprehensive therapy services. You will participate in planning, executing, and documenting treatment to improve daily living and functional abilities for patients, while educating families and coordinating with the care team.

The role requiresMichigan OTA/OTR credentials, BLS certification, and a valid MI driver’s license.

Qualifications

• Associate’s degree from accredited therapy assistant program.
• LIC-Occupational Therapist Assistant (OTA) - State of Michigan Upon Hire.
• CRT-Occupational Therapist Assistant, Registered (OTR) - NB-COT.
• LIC-Driver’s License - State of Michigan Upon Hire.
• CRT-BLS (AHA 90 Days) or ARC (90 Days) required.

Responsibilities

• Assist Occupational Therapists in providing therapy to patient/client as part of the plan of care.
• Provide treatment per plan to restore and maximize function.
• Document patient activities per department standards and regulatory requirements.
• Communicate with health-care team to coordinate care; participate in interdisciplinary conferences.
• Alert supervising OT to health changes; contribute to education for families/caregivers.
• Instruct rehabilitation staff and students about patient needs and services.
• Support program development and process improvement initiatives.
• Transport patients when needed for home care-related activities using personal vehicle.

Job description

Job Summary

Under the direction and supervision of the Occupational Therapist, provides occupational therapy services to patients/clients to assist in improving basic motor functions, reasoning abilities, compensating for permanent loss of function by teaching patients/clients to perform tasks in their daily living and working environments. May provide treatment to patients/clients with cognitive impairment, psychosocial dysfunction and physical disability. Assists with patient/clients and caregiver education.

Essential Functions

• Assists Occupational Therapists in providing therapy to patient/client. Participates in the preparation and execution of the patient/client's overall plan of care
• Provides a treatment program as planned, delegated, and supervised by the Occupational Therapist according to agency policy to restore and return maximal function to the patient/client. Assesses the effectiveness of treatment and modifies treatment methods under the direction of the Occupational Therapist
• Documentatoin for each patient/client is in compliance with department standards, medical-legal, regulatory and reimbursement procedures. Ensures that required documentatoin for activities within areas of responsibility is completed in an accurate and timely manner
• Communicates with other members of the health care team in order to ensure continuity of care and coordination of services. Attend and participate in interdisciplinary conferences to coordinate patient/client care. Provides input into education information for family or caregivers regarding the Occupational Therapy program
• Alerts the supervising Occupational Therapist or designees of potential health problems or changes in the patient/clients condition
• May instruct and train rehabilitation staff, nursing, other medical staff and students regarding patient/client needs and rehabilitation services
• Participates in ongoing process improvement and program development
• Provides transportation of the patient/client as needed using the employees personal vehicle to community based resources related to the patient/clients therapeutic goals (Home Care business lines only/based on CH entity)
