Creekside Transitional Care & Rehab in Meridian, ID is seeking an Occupational Therapy Assistant (PRN) to join our post-acute care team and provide skilled rehabilitation to residents. Responsibilities include screening and evaluating patients, developing care plans, delivering therapies, administering wound care and modalities, and preparing comprehensive discharge summaries in compliance with regulatory standards.

Open to COTAs at all experience levels; new grads welcome.

Qualifications

• Occupational Therapy Assistant license is required.

Responsibilities

• Screen and evaluate patients with physical functioning disorders and wound care to develop appropriate care plans.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions.
• Assist with standing, walking, lifting, transferring, and repositioning as part of patient care.
• Administer procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other providers.
• Generate comprehensive discharge summaries ensuring regulatory and clinical compliance.

OCCUPATIONAL THERAPY ASSISTANT - PRN

Creekside Transitional Care & Rehabilitation is a post-acute care facility in Meridian, ID, that offers long-term care, short-term care, and geriatric services.

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.
