Job summary

The Christ Hospital Home Health Care seeks an Occupational Therapy Assistant I (OTA I) to provide skilled OT services in patients' homes under the physician's plan of care. You will educate patients and families, document thoroughly, and help improve functional status while ensuring safety.

The role requires an active OTA license in Ohio, and prior acute care experience is valued. A valid driver's license is preferred to travel to multiple home visits across the territory.

Qualifications

• Active OTA license in Ohio.
• Minimum of one year OTA experience in an acute setting.
• Home care experience desired.

Responsibilities

• Provides skilled OT services in a patient’s home according to the plan of care.
• Observes, records, and reports patient progress and changes to the physician and team.
• Guides the patient in therapeutic and self-care activities to improve function.
• Educates patient and family on treatment procedures, safety measures, and proper equipment use.
• Completes and maintains clinical documentation per agency and Medicare/Federal guidelines.
• Maintains active OTA license and coordinates visits with scheduler.

The OTA I educates patients and their family members and ensures the safety of the patient. The OTA I is accountable for completing accurate documentation and remains compliant with all legal rules and regulations.
