Job summary

Alternate Solutions Health Network is seeking a part-time Occupational Therapist to provide skilled home-based therapy three days a week in the Dover to Massillon area, with occasional travel to Canton and nearby towns as needed. You will assess patients, develop and adjust plans of care, educate patients and families, and document in accordance with Medicare guidelines.

Qualifications include OT licensure in Ohio, at least one year of acute-care OT, and a valid driver’s license.

• Minimum of one year of experience as an OT in an acute setting is required.
• Home care experience is desired.
• Must have a valid driver’s license and auto insurance in your name.
• Ability to communicate effectively with patients, families, physicians and coworkers.
• Detail-oriented and observant; able to prioritize and be timely.

Responsibilities

• Provides skilled occupational therapy in patients’ homes as ordered by the physician.
• Evaluates functional level and develops a plan of care.
• Educates patients and families on therapy and home exercise programs.
• Documents treatment and coordinates with care teams per guidelines.
• Promotes continuity of care across admissions, transfers and discharges.
• Assists or supervises therapy aides as appropriate.

The OT I educates patients and their family members and ensures the safety of the patient. The OT I is accountable for completing accurate documentation and remains compliant with all legal rules and regulations.
