Job summary

Fletcher Rehabilitation and Healthcare Center in Fletcher, NC is hiring an Occupational Therapist to join our long-term care team. You will evaluate residents, design and implement therapy plans, and work with an interdisciplinary group to promote functional independence.

The role emphasizes accurate documentation, adherence to physician orders, and ongoing communication with patients and families. AOTA-recognized OT degree and NC licensure are required; SNF experience preferred.

Qualifications

• Licensed Occupational Therapist in NC with current registration.
• Graduate of an accredited OT program recognized by AOTA.
• SNF experience preferred.

Responsibilities

• Provide comprehensive OT evaluations based on MD orders.
• Develop and document treatment plans with goals, frequency and modalities.
• Ensure orders and documentation compliance for evaluations, treatments, recertifications, and discharges.
• Deliver therapy to residents and adjust plans as needed.
• Maintain accurate daily progress notes and monthly treatment plan updates.
• Collaborate with the interdisciplinary team and participate in meetings.
• Co-sign notes and supervise junior staff as needed.
• Maintain confidentiality and adhere to facility policies.
