Talented Medical Solutions in Fremont, CA is seeking a compassionate Occupational Therapist to join a 126-bed Skilled Nursing Facility on a permanent, full-time basis. The role offers a Monday–Friday schedule with no weekend rotation.

You will evaluate patients, develop and implement individualized treatment plans, and collaborate with an interdisciplinary team to promote functional independence and quality of life. This position provides stable weekday hours and opportunities to grow clinically.

Qualifications

• Graduate of an accredited Occupational Therapy program
• Current California Occupational Therapist license or eligibility for licensure
• Strong communication, organizational, and clinical skills
• Commitment to patient-centered care
• Ability to work effectively within an interdisciplinary team
• SNF experience is preferred but not required

Responsibilities

• Perform comprehensive occupational therapy evaluations and assessments
• Develop individualized treatment plans based on patient needs, abilities, and goals
• Provide skilled OT interventions to improve functional independence and quality of life
• Address activities of daily living, mobility, coordination, strength, and other functional needs
• Monitor patient progress and modify treatment plans when appropriate
• Maintain accurate and timely clinical documentation
• Collaborate with nursing, rehabilitation, and other interdisciplinary team members
• Educate patients, families, and caregivers regarding therapeutic strategies and techniques
• Participate in care planning and contribute to overall patient outcomes
• Follow facility policies, clinical standards, and applicable regulatory requirements
