Job summary

BAYADA Home Health Care is seeking an Occupational Therapist to join the Sierra Vista Visits team in Arizona. The OT will craft individualized treatment plans, work with a multi-disciplinary team, and deliver home health care to enable clients to remain at home.

Responsibilities include conducting assessments, implementing therapeutic exercises, wound care, and ongoing plan revisions. Travel within the assigned territory is required; EMR documentation is part of daily practice.

Qualifications

• Arizona OT license must be current.
• Graduate of ACOTE-accredited OT curriculum.
• Willingness to travel within assigned territory.
• Experience with EMR systems preferred.

Responsibilities

• Perform diagnostics and measurements of mobility, strength, balance, and safety.
• Develop and implement individualized OT care plans with therapeutic exercises.
• Apply wound care and integumentary interventions using evidence-based dressings.
• Continuously assess and revise the OT care plan.
• Educate clients and families on rehabilitative care and independence.
• Document observations and interventions using a tablet EMR.

BAYADA Home Health Care is currently seeking an Occupational Therapist (OT) to join our Sierra Vista Visits team in Arizona. OT’s work with clients in their home, craft treatment plans, and apply innovative techniques to help them reach their goals. You will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to remain at home. How You’ll Make an Impact

• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance, balance testing, and safety assessments
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions
• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence‑based dressings and topical agents
• Continually assess and revise the Occupational Therapy - OT care plan
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided, utilizing a state‑of‑the‑art touch pad tablet

Qualifications for a BAYADA Occupational Therapist

• A current Occupational Therapist - OT license in Arizona
• Must be willing to travel in assigned territory
• Have graduated from an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma
• Solid computer skills; prior experience with electronic medical records (EMR) preferred
• I year of experience as an Occupational Therapist
