Job summary

BAYADA Home Health Care, Inc. is seeking an Occupational Therapist (OT) to join the Yavapai Visits team in Prescott, Arizona.

You will work with clients in their homes, craft treatment plans, and apply innovative techniques to help them reach their goals as part of a multidisciplinary team providing skilled nursing and rehabilitative care. Thrive in a culture that values compassion, excellence, and reliability.

Qualifications

• AZ OT license is required.
• Willingness to travel within assigned territory.
• Graduated from an OT curriculum accredited by AMA/AOTA.
• Solid computer skills; EMR experience preferred.
• 1 year of OT experience

Responsibilities

• Perform diagnostic tests and measurements (mobility, joint ROM, transfer status, balance, strength, endurance).
• Develop and implement individualized care plans (manual therapy, gait, balance).
• Implement wound care and use evidence-based dressings and topical agents.
• Continuously assess and revise OT care plans.
• Educate clients and families in rehabilitative care and activities for health and independence.
• Document observations and interventions in a state-of-the-art tablet.

BAYADA Home Health Care is currently seeking an Occupational Therapist (OT) to join our Yavapai Visits team in Prescott, Arizona. OT’s work with clients in their home, craft treatment plans, and apply innovative techniques to help them reach their goals. You will be an integral member of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to clients, affording them the opportunity to remain at home. How You’ll Make an Impact:

• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance, balance testing, and safety assessments

• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions

• Implement integumentary protective interventions to prevent wound formation and promote wound repair utilizing evidence-based dressings and topical agents

• Continually assess and revise the Occupational Therapy - OT care plan

• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.

• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided, utilizing a state-of-the-art touch pad tablet

Qualifications for a BAYADA Occupational Therapist:

• A current Occupational Therapist - OT license in Arizona

• Must be willing to travel in assigned territory

• Have graduated from an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma

• Solid computer skills; prior experience with electronic medical records (EMR) preferred

• I year of experience as an Occupational Therapist
