Ultimate Therapy Services is seeking an Occupational Therapist (OT) to join Autumn Lake at Salem County's rehabilitation team in Salem, NJ. This full-time role focuses on helping residents regain independence and improve daily functioning within a supportive interdisciplinary setting in a skilled nursing and post-acute care facility.

Qualifications

• Active New Jersey Occupational Therapist license required
• Graduate of an accredited Occupational Therapy program
• Strong clinical, communication, and organizational skills
• Skilled nursing or rehabilitation experience preferred, but not required
• New graduates are welcome to apply

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans
• Provide skilled interventions focused on ADLs, strength, coordination, balance, cognition, and functional mobility
• Help residents improve independence with everyday tasks and routines
• Recommend adaptive equipment and educate residents and caregivers on proper use
• Monitor progress and adjust treatment plans based on resident needs
• Complete accurate and timely clinical documentation
• Collaborate with PT, SLP, nursing, and other members of the care team
