Responsibilities include evaluating ADLs, developing treatment plans, recommending adaptive equipment, and collaborating with healthcare teams to ensure optimal outcomes for post-acute care patients.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current OT license in the state of practice.
• Experience in rehabilitation, SNF, home health, or post-acute care preferred.
• Strong problem-solving and communication skills.
• Foreign-trained applicants must possess credentials deemed equivalent by the applicable credentialing organization.

Responsibilities

• Evaluate patients' ability to perform activities of daily living (ADLs).
• Develop treatment plans that enhance independence and functional performance.
• Recommend adaptive equipment and environmental modifications.
• Address cognitive, physical, and sensory limitations impacting daily function.
• Document patient progress and treatment outcomes.
• Collaborate with interdisciplinary healthcare teams.
