This position involves delivering high-quality post-acute care, rehabilitation, and therapy services to help patients regain independence. Responsibilities include ADL assessments, developing plans to improve function, and collaborating with care teams to achieve optimal outcomes.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Occupational Therapist license in the state of practice.
• Experience in rehabilitation, SNF, home health, or post-acute care preferred.
• Strong problem-solving and communication skills.
• Foreign-trained applicants must possess credentials deemed substantially equivalent by the applicable credentialing organization.

Responsibilities

• Evaluate patients' ability to perform activities of daily living (ADLs).
• Develop treatment plans that enhance independence and functional performance.
• Recommend adaptive equipment and environmental modifications.
• Address cognitive, physical, and sensory limitations impacting daily function.
• Document patient progress and treatment outcomes.
• Collaborate with interdisciplinary healthcare teams.
• Educate patients and caregivers on functional strategies for success.
