Job summary

Enhance Rehabilitation in Illinois is seeking an Occupational Therapist to evaluate and treat patients in a Skilled Nursing Facility setting, following individualized plans of care and measurable goals.

You will collaborate with facility staff to ensure excellent patient care, educate patients and families on injuries and progress, and complete timely documentation. The role includes supervising Occupational Therapy Assistants and contributing to a growing team.

Qualifications

• Licensed Occupational Therapist currently or eligible to practice.
• Experience in SNF settings is advantageous.
• Strong communication and documentation skills.

Responsibilities

• Evaluate new patients and develop plans of care with measurable goals.
• Treat patients and educate them on injury, progress and outcomes.
• Re-evaluate patient progress and adjust plans of care as needed.
• Complete timely documentation including evaluations, re-evaluations, discharges, and daily progress notes.
• Supervise Occupational Therapy Assistants and collaborate with facility staff.
