Job summary

Mohawk Meadows Rehabilitation & Nursing Center in Lafayette, NJ is seeking a skilled Occupational Therapist to evaluate residents, design individualized treatment plans, and guide recovery after injury, illness, surgery, or age-related decline. You will help residents regain independence and improve daily living activities.

You’ll document evaluations and progress, adhere to requirements, and work with an interdisciplinary team and families to support resident goals.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Active New Jersey OT license (NBCOT certification) required.
• Experience in skilled nursing or long-term care preferred.
• Strong clinical judgment, communication, and documentation skills.
• Patient-centered, team-oriented approach.

Responsibilities

• Evaluate residents’ functional abilities and develop individualized OT treatment plans.
• Deliver skilled OT services in line with established standards of practice and the resident’s plan of care.
• Help residents build or regain skills for daily living and independence.
• Document evaluations, progress, and treatment accurately and in compliance with federal, state, and facility requirements.
• Collaborate with the interdisciplinary team, physicians, and families to support resident goals.
• Supervise and direct COTAs as applicable.
