Job summary

Mohawk Meadows Rehabilitation & Nursing Center in Lafayette, NJ is seeking a skilled Occupational Therapist to help residents regain independence and improve daily quality of life. You will evaluate residents, design individualized treatment plans, and guide recovery after injury or illness, while collaborating with an interdisciplinary team and families to support goals.

A NJ OT license and NBCOT certification are required.

Qualifications

• Required degree in Occupational Therapy from an accredited program.
• Current New Jersey OT license and NBCOT certification.
• Experience in skilled nursing or long-term care preferred.
• Strong clinical judgment, communication, and documentation skills.
• Patient-centered and team-oriented approach.

Responsibilities

• Evaluate residents and develop individualized OT treatment plans
• Deliver OT services per standards and care plans
• Help residents regain daily living skills and independence
• Document evaluations, progress, and treatment according to regulations
• Collaborate with interdisciplinary team, physicians, and families
• Supervise and direct COTAs as applicable
