Job summary

The Chateau at Brooklyn Rehabilitation & Nursing Center in Brooklyn, NY is seeking an Occupational Therapist for an on-site role. You will work with an interdisciplinary team to design and implement client-centered therapy plans in a busy rehabilitation setting.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Valid Occupational Therapist license and registration in the state of practice, or license eligible.
• Ability to supervise Occupational Therapy Assistants in accordance with the state practice act.
• Comfort with clinical documentation including evaluations, weekly progress notes, recertifications, and discharge summaries.
• Commitment to confidentiality, HIPAA, and Federal Resident Rights.

Responsibilities

• Evaluate referred residents to determine medical status, functional ability, limitations, precautions, and rehabilitation potential.
• Build individualized treatment plans with measurable, achievable goals based on capacity and tolerance.
• Deliver skilled occupational therapy interventions focused on ADL performance, safety, and return to prior level of function.
• Complete evaluations, weekly progress notes, recertifications, and discharge documentation on time.
• Educate residents, families, and caregivers on therapy techniques, safety, and carryover strategies.
• Collaborate with PT, SLP, nursing, dietitians, and social work to deliver patient-centered care.
• Recommend adaptive equipment and environmental modifications.
• Supervise Occupational Therapy Assistants and students as per state rules.
