The Chateau at Brooklyn Rehabilitation & Nursing Center in Brooklyn, NY is seeking an Occupational Therapist to join our interdisciplinary team. You will evaluate residents, develop and implement individualized treatment plans, and deliver skilled OT focused on ADL and safety.

You will also complete documentation, educate families, collaborate with PT/SLP/nursing, and supervise assistants. We offer mentorship, a supportive leadership team, and opportunities for growth within a 35-facility

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Valid Occupational Therapist license and registration in the state of practice, or license eligible.
• Ability to supervise Occupational Therapy Assistants in accordance with the state practice act.
• Comfort with clinical documentation including evaluations, weekly progress notes, recertifications, and discharge summaries.
• Commitment to confidentiality, HIPAA, and Federal Resident Rights.

Responsabilités

• Evaluate referred residents to determine medical status, functional ability, limitations, precautions, and rehabilitation potential
• Build individualized treatment plans with measurable, achievable goals based on each resident’s capacity and tolerance, under physician direction
• Deliver skilled occupational therapy interventions focused on ADL performance, safety, and return to prior level of function
• Complete evaluations, weekly progress notes, recertifications, and discharge documentation accurately and on time
• Educate residents, families, and caregivers on therapy techniques, safety, and carryover strategies
• Collaborate with PT, SLP, nursing, dietitians, and social work to deliver patient centered care and support discharge readiness
• Recommend adaptive equipment, durable medical equipment, and environmental modifications
• Supervise Occupational Therapy Assistants and students in accordance with the state practice act

Formation

Degree in Occupational Therapy from an accredited program
