Job summary

Vasona Creek Healthcare in Los Gatos/South Bay is seeking a Physical Therapist to join our short-term rehabilitation team. You will work in a brand-new therapy space with advanced tools, delivering evidence-based care to help patients regain strength, mobility, and independence.

We support collaboration among PTs, PTAs, OTs, and SLPs, with a focus on patient outcomes. Immigration sponsorship and relocation bonus options are available; new grads are welcome to apply.

Qualifications

• Current California Physical Therapist (PT) license required.
• Previous experience in skilled nursing, post-acute, or rehabilitation settings preferred.
• Strong clinical reasoning and communication skills.
• Passion for rehabilitation and helping patients achieve meaningful functional outcomes.
• Commitment to evidence-based, patient-centered care.
• Ability to work collaboratively as part of a multidisciplinary rehabilitation team.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized, evidence-based treatment plans.
• Provide skilled therapy services to short-term rehabilitation and long-term care residents.
• Treat patients recovering from joint replacements, cardiac events, strokes, and other medical conditions.
• Utilize specialized rehabilitation programs, including Acute Neuro Rehabilitation and Pain Management.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with PTAs, OTs, SLPs, nursing, and other members of the interdisciplinary team.
• Educate patients and families regarding treatment plans, exercises, mobility, and strategies for continued progress.
• Promote safe mobility, functional independence, and successful discharge planning.
• Maintain accurate and timely clinical documentation.
• Advocate for patients and support their individual rehabilitation goals.
