Job summary

Sweetwater Care is seeking an Occupational Therapist to evaluate and treat patients, develop individualized plans, and educate patients and families on exercises and strategies to improve mobility and reduce pain. The role includes documenting progress, coordinating care with the rehabilitation team, and ensuring compliance with regulatory standards.

The OT will supervise PT assistants and support staff, maintain accurate records, and participate in trainings.

Qualifications

• Licensed Occupational Therapist with active state licensure.
• Ability to supervise PT assistants and rehabilitation aides.
• Provides patient counseling and education to patients and families.

Responsibilities

• Consult with patients to assess condition and plan treatment.
• Develop individualized treatment plans and educate patients on exercises and strategies.
• Document progress and maintain accurate records per regulatory standards.
• Coordinate care with rehabilitation team and other providers.
• Provide discharge summaries and ongoing education to patients and families.
• Supervise PT assistants and rehabilitation staff as needed.
