Job summary

Sweetwater Care is seeking an Occupational Therapist to design and implement treatment plans for patients recovering from illness or injury. You will collaborate with physicians and care teams, supervise assistants, and document progress and outcomes in the patient record.

You will develop individualized therapies, educate patients and families, and provide discharge summaries per regulations. Active licensure is required, and adherence to policies is essential.

Qualifications

• Active state licensure required.
• Ability to develop individualized treatment plans.
• Experience coordinating with care teams.

Responsibilities

• Consult with patients to assess their condition and review medical history.
• Diagnose movement dysfunction through observation and patient feedback.
• Develop individualized treatment plans.
• Coach patients and teach therapeutic massage techniques to ease pain.
• Maintain patient progress in medical records.
• Use standardized assessments and evidence-based practice.
• Adjust treatments based on progress and referrals.
• Provide discharge summaries per regulatory requirements.
• Document regulatory information in electronic records.
• Participate in trainings, in-services and conferences.
• Supervise and support PTAs and rehabilitation aides.
• Adhere to company policies and code of conduct.
• Maintain active State Licensure.
