Job summary

Northwell Health in New York seeks a licensed Occupational Therapist to evaluate and treat patients under physician guidance. The role focuses on goal-driven rehab using evidence-based modalities and patient-centered care.

The successful candidate will develop individualized treatment plans, collaborate with an interdisciplinary team, and educate patients and caregivers in daily living activities to maximize independence and function.

Qualifications

• Bachelor’s Degree in Occupational Therapy, required; Master’s Degree preferred.
• Minimum one (1) year prior experience, required.
• Must be currently licensed as Occupational Therapist in New York State.

Responsibilities

• Evaluates and treats patients with disabilities, injuries and/or disease in accordance with the prescription/referral.
• Administers direct patient care and submits required documentation per policies.
• Designs and implements treatment plans to meet patient goals and improve function.
• Collaborates with interdisciplinary team to maximize patient recovery and independence.
• Instructs patients and caregivers in daily living activities and safe transfers.
• Reviews quality improvement activities and coordinates with care teams.
• Assesses need for DME and provides follow-up care recommendations upon discharge.
