Job summary

Community First Solutions’ Jamestowne short-term rehab facility seeks a Licensed Occupational Therapist to evaluate residents, plan and implement therapy, and discharge planning. The OT will collaborate with residents, families, and an interdisciplinary team to enhance independence and daily functioning.

Ohio license and CPR are required, with at least two years’ experience in long-term care preferred. You’ll work in a mission-driven environment with hands-on support, ongoing professional

Qualifications

• Bachelor’s degree in an accredited OT program (Master’s degree preferred).
• Current Ohio OT license and CPR certification required.
• At least two years’ experience, preferably in long-term care.
• Strong evaluation, treatment planning, documentation and collaboration skills.

Responsibilities

• Administers OT services following standards and policies.
• Conducts evaluations, plans care, and discharge orders.
• Collaborates with residents, families, and staff to optimize rehabilitation.
• Ensures safety and OSHA compliance and guides COTAs.
