Job summary

FutureCare Health & Management is seeking an Occupational Therapist to join our interdisciplinary rehabilitation team in Maryland. The role emphasizes evaluating residents, developing individualized treatment plans, and providing ongoing assessment and documentation to maximize residents’ functional abilities.

You will supervise assistants, coordinate with nursing staff and families, and ensure compliance with facility policies and state/federal regulations.

Qualifications

• Current Occupational Therapist license/certification in Maryland.
• Minimum of one year experience in rehabilitation.
• Supervision or training experience in long-term care settings preferred.

Responsibilities

• Review available resident information and evaluate the impact on assessment and treatment.
• Conduct and document a thorough functional assessment using standardized procedures.
• Provide assessment of home/vocational and equipment needs as suggested.
• Develop and implement an individualized treatment plan with achievable functional goals.
• Comply with evaluation, treatment and documentation guidelines defined by policy and regulations.
• Document the course of treatment in the resident’s medical record after each session.
• Revise treatment plans as needed throughout the treatment process.
• Instruct assistants in treatment procedures and provide supervision.
• Orient residents to program services and inform about risks during procedures.
• Collaborate with nursing staff, residents and families to maximize functional abilities.
• Instruct residents and families in exercises to continue post-discharge.
• Assist in referrals for additional services after discharge.
• Maintain knowledge of community resources and participate in meetings.
• Assist Rehab Director with feedback on clinical performance as requested.
