Job summary

SanStone Health & Rehabilitation is seeking an Occupational Therapist to provide comprehensive therapy in a skilled nursing facility in North Carolina. You will assess residents, plan individualized OT programs, and collaborate with a multidisciplinary team to improve ADLs and overall independence.

Responsibilities include documentation, discharge planning, and family education. Applicants should hold a NC OT license with a Master’s or Doctoral degree, and new graduates are encouraged to apply.

Qualifications

• Approved OT licensure required in North Carolina.
• Strong assessment and treatment planning skills.
• Experience in skilled nursing or long-term care preferred, new graduates welcome.

Responsibilities

• Assess residents' physical, cognitive and emotional needs.
• Develop and implement individualized OT treatment plans focusing on ADLs.
• Collaborate with nurses, PTs, SLPs, and social workers.
• Document evaluations, plans, progress, and discharge summaries.
