Job summary

The Legacy at Boca Raton Rehabilitation and Nursing Center in Boca Raton, FL, is seeking a dedicated Occupational Therapist (OT) to evaluate residents and develop individualized treatment plans aimed at improving daily living skills, independence, and quality of life. This role involves collaborating with physicians, nursing staff, and the interdisciplinary team to support positive resident outcomes.

Qualifications

• Active Florida OT license in good standing.
• Experience in skilled nursing or rehabilitation settings preferred.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and interpersonal abilities.
• Ability to work effectively within an interdisciplinary team.

Responsibilities

• Evaluate residents and develop individualized occupational therapy plans.
• Provide interventions to improve strength, coordination, mobility and daily living activities.
• Assess progress and modify treatment plans as appropriate.
• Collaborate with physicians, nursing staff, and the interdisciplinary team.
• Maintain accurate clinical documentation per regulatory standards.
• Educate residents and families about therapy programs and goals.
• Participate in care planning meetings and discharge planning.
• Ensure compliance with federal, state, and facility policies.
