The Legacy at Boca Raton Rehabilitation and Nursing Center in Boca Raton, FL, is seeking a dedicated Occupational Therapist (OT) to join our rehabilitation team. The OT will evaluate residents and develop individualized treatment plans to improve daily living skills and functional independence.

Qualifications

• Active Florida OT license in good standing.
• Experience in a skilled nursing facility or rehabilitation setting preferred.
• Strong clinical assessment and treatment planning skills.
• Excellent communication and interpersonal abilities.
• Ability to work effectively within an interdisciplinary team.
• Compassionate, dependable, and resident-focused.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide therapeutic interventions to improve strength, coordination, mobility, and activities of daily living.
• Assess resident progress and modify treatment plans as appropriate.
• Collaborate with physicians, nursing staff, and interdisciplinary team members to support positive resident outcomes.
• Maintain accurate and timely clinical documentation in accordance with regulatory and facility standards.
• Educate residents and families regarding therapy programs, adaptive equipment, and treatment goals.
• Participate in care planning meetings and discharge planning processes.
• Ensure compliance with all federal, state, and facility policies and procedures.
• Maintain a safe, organized, and supportive therapy environment.
