Job summary

The Legacy at Boca Raton Rehabilitation and Nursing Center in Boca Raton, FL, is seeking a dedicated Occupational Therapist (OT) to join our rehabilitation team. You will evaluate residents, develop individualized treatment plans, and provide interventions to improve daily living skills, mobility, and independence.

Qualified candidates hold an active Florida OT license and have experience in skilled nursing or rehab settings.

Qualifications

• Active Florida OT license in good standing.
• Previous experience in a skilled nursing facility or rehabilitation setting preferred.
• Strong clinical assessment and treatment planning skills.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide therapeutic interventions to improve strength, coordination, mobility and activities of daily living.
• Assess resident progress and modify treatment plans as appropriate.
• Collaborate with physicians, nursing staff and interdisciplinary team members to support positive resident outcomes.
• Maintain accurate and timely clinical documentation.
• Educate residents and families regarding therapy programs, adaptive equipment and treatment goals.
• Participate in care planning meetings and discharge planning processes.
• Ensure compliance with all policies and procedures.
