Qualifications

Responsibilities

• Initiates evaluations per MD orders and facility guidelines.
• Develops and implements treatment plans for optimal outcomes.
• Educates staff and third-party payers on benefits and outcomes.
• Completes documentation including evaluations and progress notes.
• Enters data into EMR and rehab systems.
• Screens patients who may benefit from therapy.
• Ensures safety of self and patients at all times.
• Drives interdisciplinary care and program development.
• Attends continuing education and provides in-service training.
• Supervises assistants and rehab aides as applicable.
• Maintains licensure and credentialing.
• Submits billing and payroll documentation.
• Makes referrals to rehab team.
