Aspire Home Healthcare Of Northern California Inc is seeking a part-time Occupational Therapist to provide per-visit therapy in the Redding area. This role focuses on individualized patient care and collaborative treatment planning within a total rehabilitation program.

Responsibilities include evaluating patients, developing goals, supervising COTAs and HHAs, documenting progress, and coordinating care with the team.

Qualifications

• Licensed OT in CA is required.
• Minimum of 1 year OT experience preferred.
• Current TB results within 12 months and CPR Certification.
• Valid driver's license and proof of liability insurance on automobile.
• Ability to attend 1-day orientation at our Yuba City office.
• OASIS and WellSky (Kinser) experience strongly preferred.

• Evaluate patient’s condition, functioning level, and goals; plan and carry out treatment.
• Develops and supervises COTAs and HHAs per treatment plan.
• Documents patient treatment in medical chart daily; submits notes/visits timely.
• Provides therapy services as outlined in Plan of Care.
• Coordinates total patient care and discharge planning as needed.
• Maintains HIPAA confidentiality and adheres to regulations.

OT - Occupational Therapist- part-time needed!This is a paid per visit position!Aspire Home Healthcare was provides patients with around the clock care and support. Assigned patient’s capabilities and disabilities, plans and carries out a progressive treatment program, and coordinates treatment program with total rehabilitation program.Duties Include:· Effective patient care using approved treatment techniques and procedures. Evaluation of patient’s condition, functioning level and treatment goals and keeping current records.· After evaluating the patient, the therapist is responsible for planning administering and supervising the COTAs and HHAs in accordance with the treatment plan.· Administers appropriate evaluations to assess patient’s level of function and develops therapy goals. Develops treatment plan for each patient and performs appropriate individual treatment of assigned patients. Documents patient’s treatment in medical chart daily.· Will provide therapy services as outlined in the Plan of Care.· Assists patient and family with development and review of home program, evaluation and review of adaptive equipment needed in home setting and evaluation of onsite home setting when necessary.· Charts services the Therapist has rendered to the patient, notes changes in the patient’ condition and/or family and home situation, makes revisions in the care plan as needed, records supervisory visits, evaluates patient care and progress and closes charts of discharged patients by preparing Discharge Summaries and obtaining Discharge orders.· Maintains confidentiality of client’s situation, records and services provided according to HIPPA regulations· Evaluates the effectiveness of his/her therapy service to the individual and family.· Submits all patient notes and/or visit records on a timely basis.· Obtains orders for paramedical services and submits referral to the Director of Clinical Services, i.e., nursing services, medical social services, etc.· Coordinates total patient care.Qualifications Include:· Licensed OT in CA.· Minimum of 1 year as an OT experience preferred.· OASIS and Wellsky (Kinser) experience strongly preferred.· Physical within 6 months prior to first patient contact.· Current TB results (within last 12 months) and CPR Certification.· Valid driver’s license and proof of liability insurance on automobile.· Ability to attend 1-day orientation at our Yuba City office.Work Location: In person
