Job Overview

Aspire Home Healthcare of Northern California provides around‑the‑clock care and support to patients. This part‑time occupational therapist position is designed to deliver services in the Sacramento area. The role includes evaluating, planning, and delivering patient‑centered therapy, documenting care, and coordinating with other clinicians.

Responsibilities

• Provide effective patient care using approved treatment techniques and procedures, evaluating the patient’s condition, function level, treatment goals, and maintaining records.
• Plan, administer, and supervise COTAs and HHAs in alignment with the treatment plan.
• Conduct evaluations to assess functional level, develop therapy goals, and create individualized treatment plans, documenting daily in the medical chart.
• Administer therapy services as outlined in the Plan of Care.
• Assist patients and families with developing and reviewing home programs, evaluating adaptive equipment needs, and reviewing onsite home environments.
• Chart services rendered, note changes in condition or home situation, revise care plans as needed, record supervisory visits, evaluate care and progress, and close charts of discharged patients with summary reports.
• Maintain confidentiality of client records according to HIPAA regulations.
• Evaluate the effectiveness of therapy for patients and families.
• Submit all patient notes and visit records promptly.
• Obtain orders for paramedical services and submit referrals to the Director of Clinical Services (nursing, medical social services, etc.).
• Coordinate total patient care.

Qualifications

• Licensed occupational therapist in California.
• Minimum of 1 year OT experience (preferred).
• Experience with OASIS and Wellsky (Kinser) strongly preferred.
• Physical exam cleared within 6 months prior to first patient contact.
• Current TB results (within last 12 months) and CPR certification.
• Valid driver’s license and proof of liability insurance for automobile.
• Availability to attend a 2–3 day orientation at our Yuba City office.
