Job summary

Snap Health in Texas is seeking a motivated Independent Contractor Occupational Therapist (OT) to support home health patients with flexible scheduling.

You will assess functional status, develop and implement care plans, coordinate with the health team, and participate in discharge planning while supervising OTA’s. Licensure in Texas and 1+ year OT experience are required.

Qualifications

• Valid Occupational Therapist license in Texas.
• Minimum of 1 year of experience in Occupational Therapy and mild exposure to home health.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Proficient in using mobile apps for organizational purposes.

Responsibilities

• Evaluate functional status and therapy needs; assist plan of care.
• Provide OT services within scope of practice and plan of care.
• Treat patients to relieve pain and restore/maintain function.
• Instruct patient and family about plan of care and progress.
• Visit patients per Plan of Care and complete progress notes.
• Provide supervision of OTA’s.
• Participate in discharge planning.

Independent Contractor Occupational Therapist (OT) Essential Job Functions/Responsibilities

• Evaluate the patient’s functional status and physical therapy needs. Assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by the state board of occupational therapy, in accordance with the plan of care, and in coordination with other members of the health care team.
• Treats patients to relieve pain, restore function, and / or maintain maximum functioning using acceptable standards of practice.
• Instructs and informs participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
• Visits patients according to Plan of Care, completes progress notes timely.
• Provides supervision of OTA’s
• Participates in discharge planning for patient.
