Job summary

Oakmont Healthcare is seeking a compassionate Occupational Therapist (OT) to join our in-house rehab team on a PRN basis, focusing on resident independence, safety, and quality of life through meaningful daily activities in a SNF setting in Texas.

You will evaluate patients, develop and adjust treatment plans, recommend adaptive equipment, and collaborate with an interdisciplinary team while documenting thoroughly and ensuring compliance.

Qualifications

• Active and valid Texas Occupational Therapy license.
• SNF or long-term care experience preferred.
• Experience with Net Health Electronic Documentation System preferred.
• Strong communication and collaboration skills.

Responsibilities

• Consult with patients to evaluate their physical condition and its impact on daily occupations and quality of life.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize ADL/IADL performance.
• Assess patients’ home environments and recommend modifications.
• Recommend adaptive equipment and provide training to support independence and safety.
• Ensure treatments align with the care plan and are delivered accordingly.
• Monitor patient response and modify interventions with the interdisciplinary team.
• Provide clinical education and support to patients, families, and caregivers.
• Collaborate with interdisciplinary team members to ensure high-quality care.
• Complete documentation thoroughly and timely per policies.
• Comply with applicable rules, regulations, and procedures.
• Attend required Rehab Director-designated meetings.

We are seeking a compassionate and flexible Occupational Therapist (OT) to join our in-house team of outstanding professionals on a PRN basis. In this role, you will provide person-centered care focused on improving residents’ independence, safety, and quality of life through meaningful daily activities and functional outcomes.

Your Impact as an Occupational Therapist (OT)

• Consult with patients to evaluate their physical condition and its impact on daily occupations, well-being, and quality of life
• Assess and interpret patient evaluations and test results
• Develop individualized treatment plans using a variety of therapeutic techniques to maximize performance in activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
• Assess patients’ home and living environments and recommend modifications based on their needs and capabilities
• Recommend adaptive equipment and provide training to support independence and safety
• Ensure all treatments are delivered in accordance with the established plan of care
• Monitor patient response to treatment and modify interventions as clinically appropriate in collaboration with the interdisciplinary team
• Provide clinical education and support to patients, family members, and caregivers
• Collaborate with interdisciplinary team members to ensure high-quality patient care
• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations
• Comply with all applicable rules, regulations, and procedures
• Attend required meetings as designated by the Director of Rehab

What Makes You a Great Fit

• Active and valid Texas Occupational Therapy license
• In good standing with all regulatory agencies and licensing boards
• Skilled Nursing Facility (SNF) or long-term care experience preferred
• Current knowledge of occupational therapy treatment practices
• Experience with Net Health Electronic Documentation System preferred
• Ability to manage patients with varying personalities and needs
• Strong communication, collaboration, and clinical decision-making skills
