Job summary

Park Place Care is seeking a compassionate Occupational Therapist (OT) to join our in-house rehab team on a PRN basis. You will evaluate residents, develop individualized plans, and improve independence and safety through meaningful daily activities.

You will assess home environments, recommend adaptive equipment, and work with an interdisciplinary team to deliver person-centered care, while ensuring timely documentation and compliance with state and federal regulations.

Qualifications

• Active and valid Texas Occupational Therapy license.
• In good standing with regulatory agencies and boards.
• SNF or long-term care experience preferred.
• Current knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.
• Ability to manage patients with varying personalities and needs.
• Strong communication, collaboration, and clinical decision-making skills.

Responsibilities

• Consult with patients to evaluate their condition and impact on daily activities.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize ADLs/IADLs.
• Assess home environments and recommend safety-related modifications.
• Recommend adaptive equipment and provide training for independence.
• Deliver treatments per the plan of care and adjust as needed.
• Monitor patient response and modify interventions with the team.
• Provide clinical education to patients, families, and caregivers.
• Collaborate with interdisciplinary team for high-quality care.
• Complete documentation thoroughly and timely per policies and regs.
• Comply with all applicable rules and procedures; attend meetings with Rehab leadership.

Join Our Team as a (PRN) Occupational Therapist (OT)

We are seeking a compassionate and flexible Occupational Therapist (OT) to join our in-house team of outstanding professionals on a PRN basis. In this role, you will provide person-centered care focused on improving residents’ independence, safety, and quality of life through meaningful daily activities and functional outcomes.

Your Impact as an Occupational Therapist (OT)

• Consult with patients to evaluate their physical condition and its impact on daily occupations, well-being, and quality of life

• Assess and interpret patient evaluations and test results

• Develop individualized treatment plans using a variety of therapeutic techniques to maximize performance in activities of daily living (ADLs) and instrumental activities of daily living (IADLs)

• Assess patients’ home and living environments and recommend modifications based on their needs and capabilities

• Recommend adaptive equipment and provide training to support independence and safety

• Ensure all treatments are delivered in accordance with the established plan of care

• Monitor patient response to treatment and modify interventions as clinically appropriate in collaboration with the interdisciplinary team

• Provide clinical education and support to patients, family members, and caregivers

• Collaborate with interdisciplinary team members to ensure high-quality patient care

• Complete documentation thoroughly and timely in compliance with company policies and state/federal regulations

• Comply with all applicable rules, regulations, and procedures

• Attend required meetings as designated by the Director of Rehab
