Job summary

Fairview Hospital / Cleveland Clinic in Colorado seeks an Occupational Therapist to join our in-home health team. You will evaluate clients, deliver therapy per physician orders, and coordinate care to support client independence.

The role requires CO licensure, at least one year of professional experience, and a valid driver\’s license. You will supervise therapy assistants, ensure documentation, and comply with state OT rules while pursuing ongoing professional development.

Qualifications

• Licensed or registered as an Occupational Therapist in Colorado.
• At least one year of professional experience.
• Valid driver’s license, proof of insurance, and transportation.
• Graduate from an accredited Occupational Therapy program or eligible for certification.

Responsibilities

• Conduct client evaluations and deliver Occupational Therapy based on physician orders.
