Job summary

Collage Home and Community Rehabilitation Partners is seeking an experienced Occupational Therapist to provide patient-centered care in Rancho Cucamonga, CA, and surrounding areas. This PRN position offers treatment in patients’ homes and communities with interdisciplinary teams aiding integrated plans.

Applicants should hold a Bachelor’s in OT with state licensure; Master’s degree or certification is preferred, and experience with neurologically related disorders is strongly valued.

We are looking for experienced Occupational Therapists who want to provide truly patient-centered care in Rancho Cucamonga, CA and surrounding areas - with PRN positions available. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities. Our therapists are creative in their approach and incorporate a variety of activities to ignite interest while addressing functional goals. This evidence-based approach results in durable outcomes.
