Job summary

Heartland Nursing and Rehabilitation Center in Boynton Beach, FL is seeking a compassionate Occupational Therapist (PRN) to join our rehabilitation team. Provide evaluations and individualized treatment plans to help residents regain independence and improve daily living activities.

You will collaborate with physicians, nurses, and other therapy professionals in a supportive, team-oriented environment where your expertise makes a difference.

Qualifications

• Must hold an active Florida Occupational Therapist license.
• Experience in long-term care or skilled nursing facilities preferred.
• Demonstrates ability to work well in an interdisciplinary team.

Responsibilities

• Perform comprehensive occupational therapy evaluations.
• Develop and implement individualized treatment plans.
• Utilize evidence-based interventions to improve daily living activities.
• Monitor progress and adjust plans as needed.
• Maintain accurate, timely, compliant documentation.
• Collaborate with physicians, nurses and therapy staff.
• Assist residents recovering from illness, injury, or disability.
• Promote resident-centered care and a positive environment.

Heartland Nursing and Rehabilitation Center is seeking a compassionate and motivated Occupational Therapist to join our dynamic rehabilitation team. In this role, you will help residents regain independence, improve functional abilities, and enhance their overall quality of life through individualized therapy programs.

You will collaborate closely with physicians, nurses, and other therapy professionals in a supportive, team-oriented environment where your expertise truly makes a difference.

Key Responsibilities

• Perform comprehensive occupational therapy evaluations.
• Develop and implement individualized treatment plans based on residents' functional goals.
• Utilize evidence-based therapeutic interventions to improve independence in daily living activities.
• Monitor resident progress and modify treatment plans as necessary.
• Maintain accurate, timely, and compliant documentation.
• Collaborate with an interdisciplinary team to ensure coordinated and effective care.
• Assist residents recovering from illness, injury, or disability in maximizing their functional potential.
• Promote a positive, respectful, and resident-centered care environment.

Qualifications

• Graduate of an accredited Occupational Therapy program
• Current and active Florida Occupational Therapist license
• Clear/active license and Level 2 background screening
• Experience in long-term care or skilled nursing facilities preferred
