Job summary

Evolve Therapy Services is seeking a compassionate Occupational Therapist for our in-house Skilled Nursing Facility in Fremont, OH.

The PRN position focuses on planning, directing OT services and ensuring care aligns with federal, state, and local standards. You will collaborate with the Administrator and interdisciplinary team to maintain high-quality resident care.

Qualifications

• Degree in Occupational Therapy and active state licensure required.
• Continued professional development to maintain licensure and certification.
• Ability to manage therapy services in accordance with standards.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents to develop treatment plans and goals.
• Communicate results to residents, families, and the team.
• Establish resident therapy schedules with the team.
• Deliver therapy per plan and document/bill services.
• Oversee COTA and ensure proper documentation and reimbursement.

Job Position: Occupational Therapist for an In-house Skilled Nursing Facility

Job Type: PRN

Job Purpose: To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy
• MUST have an Active Occupational Therapy license in practicing state(s)
• Ongoing continuing education and professional development to maintain current licensure and certification
