Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in Lyndhurst, OH. This full-time role requires planning, directing, and delivering comprehensive OT services to residents, with supervision of COTA as needed.

You will conduct screenings, develop treatment plans, document care, and coordinate with families and the multidisciplinary team to ensure high-quality resident outcomes and compliant reimbursement practices.

Qualifications

• Degree in Occupational Therapy is required.
• Active Occupational Therapy license in practicing state(s) is required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to identify appropriate interventions.
• Develop and monitor individualized OT treatment plans and goals.
• Interpret and communicate evaluation findings to residents, families, and the healthcare team.
• Schedule resident therapy sessions with the care team.
• Deliver therapy per plan and document/bill OT services accurately.
• Perform additional duties as needed to optimize patient care.
• Oversee COTA delivery of therapy and maintain clinical excellence and proper reimbursement.

Education

Job Position:

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type:

Full Time

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient

• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications:

• Degree in Occupational Therapy

• MUST have an Active Occupational Therapy license in practicing state(s)
