Job summary

Louisville East Post Acute is seeking a fulltime OT to join our 178-bed facility serving multiple units including sub acute rehab, Pathways, TBI, LTC and memory care. You will develop treatment plans and coordinate with referring physicians to obtain approvals for services.

As a member of our team, you will provide patient-centered care, document progress, and collaborate with interdisciplinary colleagues to support patient recovery and daily living goals.

Responsibilities

• Develop treatment plans and obtain approval for services from referring physician.
• Treat patients per the physician treatment plan.
