Job summary

Heights Care & Rehabilitation is seeking a full‑time Occupational Therapist based on site in Denver, CO. You will evaluate residents, create individualized plans, and implement evidence‑based therapies to improve independence and quality of life.

You’ll collaborate with nurses, physicians, and rehab staff to coordinate care and advance daily living skills. Documentation of assessments and outcomes will follow standard regulatory requirements, while educating residents and families on strategies

Qualifications

• Current state licensure or eligibility for licensure as an Occupational Therapist.
• Graduation from an accredited Occupational Therapy program.

Responsibilities

• Evaluate residents’ functional abilities and develop individualized treatment plans.
• Design and implement evidence-based interventions.
• Provide therapeutic activities to support daily living skills, mobility, and cognitive function.
• Collaborate with nursing, physicians and rehab staff to coordinate care.
• Document assessments, sessions and outcomes per regulatory standards.
• Educate residents, families and caregivers on strategies to optimize function and safety.
• Support quality improvement initiatives and person-centered care.
