Job summary

Visiting Nurse Association in Hyannis, MA is seeking an Occupational Therapist per visit to evaluate patients and develop individualized plans of care focused on function and safety. You will work Monday–Friday with occasional weekends/holidays and collaborate with an interdisciplinary team to optimize outcomes.

The role requires a valid Massachusetts OT license and at least one year of clinical experience, with CPR certification and solid documentation skills.

Qualifications

  • Bachelor of Science or Master’s in Occupational Therapy from an accredited program.
  • Current OT license in Massachusetts.
  • Minimum one year clinical experience.
  • CPR certification required.
  • Computer skills preferred.

Responsibilities

  • Perform a comprehensive OT assessment and develop a plan of care to promote function and safety.
  • Coordinate with the interdisciplinary team and participate in care planning.
  • Provide documented supervision of the Home Health Aide plan of care every 14 days.
  • Document admission, progress, and discharge activities clearly and timely.
  • Communicate significant changes in patient condition to the MD and care team.
  • Assist with discharge planning and patient/family teaching.

Job description

Purpose of Position

Performs a comprehensive Occupational Therapy assessment of patients referred for service including but not limited to physiological, psychosocial, environmental factors and health history. Integrates pertinent data to develop a plan of care to promote function and safety. Participates with other staff in the development of to develop a plan of care to promote function and safety. Participates with other staff in the development of agency policies and programs necessary to achieve agency objectives as they relate to patient care.

Description
  • Maintain annual HHRG values at the National Standard as appropriate.
  • Utilize HHRG values to establish visit frequency and to establish appropriate interdisciplinary utilization.
  • Annual outcomes meet National Benchmarks.
  • Refers to community resources as appropriate.
  • Evaluates the clinical needs of the patient; initiates, develops, implements and makes necessary revisions to the patient's plan of care; addresses all problems in the plan of care and documents accordingly.
  • Participates in inter-disciplinary patient case conferences; evaluates effectiveness of established plan of care.
  • Instructs and provides documented supervision of the Home Health Aide plan of care every 14 days.
  • Notifies MD/other involved care providers of significant changes in the patient’s condition.
  • Completes admission and other paperwork on a timely basis.
  • 1Records narrative documentation to reflect skilled care. Documentation is clear, concise and addresses specifics of teaching, direct care and response of patient / caregiver.
  • Obtains and clarifies valid MD orders for all care delivered.
  • Plans and coordinates discharges appropriately and informs patient/MD and records disposition of patient in the discharge summary. Completes all appropriate discharge paperwork.
  • Confers with COTA and provides written documentation regarding baseline data, treatment plan, patient status and precautions prior to initial COTA visit. Delegate patient care appropriately. Supervise and case conference with COTA according to departmental guidelines.
  • Instructs patient family, significant other, in patient's total Occupational Therapy treatment program.
  • All other duties as assigned.

Qualifications
  • Bachelor of Science Degree or Master's Degree in Occupational Therapy from an institution accredited jointly by the American Occupational Therapy Association and the committee on Allied Health Education and Accreditation of the American Medical Association.
  • Current OT license in Mass.
  • Minimum one (1) year clinical experience and CPR certification required.
  • Computer skills preferred.
Schedule Details

Occupational Therapist Per Visit, Monday–Friday, Occasional Weekends and Holidays. Area: Mid/Lower Cape
