Job summary

Visiting Nurse Association in Hyannis, MA is seeking an Occupational Therapist per visit to assess patients, develop and implement plans of care, and coordinate with a multidisciplinary team. The position requires OT licensure in Massachusetts and at least one year of clinical experience, with CPR certification. Schedule includes weekdays with some weekends/holidays as needed.

Qualifications

• Bachelor of Science Degree or Master’s Degree in Occupational Therapy from an institution accredited jointly by the American Occupational Therapy Association and the committee on Allied Health Education and Accreditation of the American Medical Association.
• Current OT license in Mass.
• Minimum one year clinical experience and CPR certification required.
• Computer skills preferred.

Responsibilities

• Performs a comprehensive Occupational Therapy assessment of patients referred for service including physiological, psychosocial, environmental factors and health history.
• Develops a plan of care to promote function and safety.
• Coordinates with interdisciplinary team and participates in policy/program development as needed.
• Instructs and supervises Home Health Aide plan of care every 14 days.
• Documents patient status, care delivered, and discharge disposition clearly and timely.
• Communicates significant changes in patient condition to MD/other providers.
• Delivers teaching to patient and family and coordinates discharge planning.
