Overview

Western Connecticut Home Care is seeking occupational therapists to join our exceptional home healthcare team. Our staff work independently at the peak of their professional license while coordinating with a team to provide care in patients’ homes. Summary

Provides services to identify, prevent, remediate, and rehabilitate acute or prolonged physical dysfunction or pain, emphasizing upper‑extremity function, maximal independence in activities of daily living, and cognitive‑perceptual motor function. Assumes responsibility for ongoing interdisciplinary assessment and development of individualized plans of care in partnership with the patient, representative (if any), and caregiver. Communicates with the responsible physician and other health‑care practitioners related to the current home‑health plan of care.

Responsibilities
  • Develop, implement, and evaluate the plan of care in partnership with the patient, representative (if any), caregiver(s), and physician(s).
  • Provide services ordered by the physician as indicated in the plan of care and communicate with all involved physicians and health‑care practitioners.
  • Deliver direct occupational therapy services in accordance with the plan of care established by the physician.
  • Maintain records of visits, including evaluation, plan of care, client response to therapy, discharge planning when applicable, and clinical notes.
  • Participate in case‑management conferences with the nurse and other team members to coordinate the occupational therapy program with the total plan of care.
  • Provide education and counseling to the patient and family members regarding the occupational therapy treatment regimen.
  • Identify patient and family needs for other home‑health services and make client referrals when needed.
  • Participate in the agency quality‑assurance and performance‑improvement program and in‑service training.
  • Supervise care provided by COTAs and home‑health aides as appropriate.
  • Re‑evaluate client progress and treatment regimen, making changes as indicated, and under physician recommendations.
  • Fulfill all compliance responsibilities related to the position, including in‑service training.
  • Perform other duties as assigned.
Requirements

Minimum Experience: One year. Desired: Experience in a qualified home‑health agency.

Working Conditions

Manual: Significant manual skills, motor coordination, and finger dexterity. Occupational: Some occupational risk. Physical effort: Very heavy effort, may exert up to 50 lbs. Physical environment: Possible exposure to dirt, odors, noise, human waste, etc. Company: Western CT Home Care Inc. Org Unit: 610. Department: Occupational Therapy. Exempt: Yes. Salary Range: $32.64 – $65.01 per hour.
