Job summary

Western Connecticut Home Care, part of the Nuvance Health Network, seeks an occupational therapist to join our home health team in Danbury, CT. You will work independently at the top of your license while collaborating with patients, families, and physicians to deliver care in the comfort of patients' homes.

Qualifications

• Licensed/eligible for occupational therapy in Connecticut.
• Minimum one year of home health experience preferred.
• Experience in a qualified home health agency is a plus.

Responsibilities

• Develops, implements and evaluates the plan of care in partnership with the patient, representative (if any), caregiver(s) and physician(s).
• Provides services ordered by the physician as indicated in the plan of care; communicates with physicians and other health care practitioners as appropriate.
• Provides direct occupational therapy services in accordance with the plan of care established by the physician.
• Maintains records on visits to clients, including evaluation, plans for care, and client response; conducts discharge planning and documents clinical notes.
• Participates in case management conferences with the nurse and other team members to coordinate OT with total plan of care.
• Provides education and counseling to the patient and family regarding occupational therapy treatment regimen.

Summary

Provides services that identify, prevent, remediate, and rehabilitate acute or prolonged physical dysfunction or pain with emphasis on upper extremity function, maximal independence in activities of daily living, and cognitive perceptual motor function. Assumes responsibility for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative (if any) and caregiver. Communicates with the physician who is responsible for the health plan of care and other health care practitioners as appropriate related to the current home health plan of care.
