Job summary

Evolve Therapy Services is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility in Erie, PA. This PRN position focuses on delivering high-quality therapy aligned with regulatory standards and facility goals.

Responsibilities include screening, evaluation, treatment planning, scheduling, documentation, and coordination with the care team. The OT must hold an active license in the practicing state and commit to ongoing professional development.

Qualifications

• Degree in Occupational Therapy required.
• Active OT license in practicing state(s) required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention for each patient.
• Evaluate residents to inform treatment plans and set short- and long-term goals.
• Interpret and communicate evaluation findings and recommendations to residents, families, and the team.
• Establish individual therapy schedules in coordination with other team members.
• Conduct therapy per plan and document and bill OT services.
• Perform other duties as needed to effectively treat patients.
• Oversee COTA and ensure documentation and reimbursement practices are followed.

Education

Degree in Occupational Therapy

Active Occupational Therapy license

Ongoing continuing education required

Job Position

Occupational Therapist for an In-house Skilled Nursing Facility

Job Type

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient

• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals

• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members

• Establish individual resident therapy schedules in cooperation with other team members

• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services

• Perform all other duties as assigned and as required to effectively treat patients

• Oversee COTA as needed and ensure all documentation and treatments are carried out with clinical excellence and proper reimbursement practices are being followed

Education & Qualifications

• Degree in Occupational Therapy

• MUST have an Active Occupational Therapy license in practicing state(s)

• Ongoing continuing education and professional development to maintain current licensure and certification
