Qualifications

• Education: Bachelor’s Degree from an Accredited Program in Occupational Therapy.
• Licensure/Certification: Licensure and Registration in Arizona. CPR Certification.
• Experience: One year.

Responsibilities

• Assess patient needs for occupational therapy in a timely fashion; completes initial evaluation in accordance with department policies.
• Develops and maintains long and short-term treatment goals that are functional and measurable for each patient; re-evaluates as warranted.
• Integrates information, involves patient/family in planning, and provides education on status recommendations.
• Utilizes the most appropriate form of treatment with the highest level of care.
• Maintains and upgrades skills through continued education and professional involvement.
